Crynodeb
To assess by telephone peoples access to community support and identify needs to prevent readmission to hospital by enabling advice and guidance to signpost to relevant services. Helping people to find and use local services that they need and if necessary, refer and follow up on referral outcomes. To provide a holistic approach to triage and open conversations around needs identified upon hospital discharges. Provide evidence-based information to people about admission avoidance related to access to community support, self-management, and general wellbeing. Undertake any other delegated work Any other function commensurate with competencies and job role
