Crynodeb
You will have the ability to provide case management of an identified caseload of children and young people with complex health needs by utilising and promoting evidence-based care. You should have excellent interpersonal skills to support you in managing complex discussions with a range of agencies and individuals, including care providers, patients and families. In addition the ability to work in partnership with parents and multi-disciplinary agencies to provide skilled care to children and young people within the community to support safe transitioning to adult health services. You will be highly organised and have a flexible approach to work, able to deal with difficult situations autonomously with demonstrable, excellent communication skills.
