Crynodeb
To be accountable and responsible for a defined caseload that reflects the priorities of the local population bycarrying responsibility for the assessment of care needs; the development, implementation and evaluation of programme of care; setting standards of care; and liaison with other agencies, particularly in relation to: Close working with other services aiming to reduce health inequalities and improve access to mainstream mental health, acutecare and primary care services. Providing an effective in reach service to people with learning disabilities who have been admitted to mental health and forensic wards, therebyfacilitating effective and timely discharge. The provision of early intervention aimed at promoting andmaintaining the physical and mental health of individuals and their families or carers. To work as a member of the multi-professional team, to attend regular team meetings, supervision forums, participate constructively in team processes and contribute positively to overall service developments. To receive referrals as an autonomous practitioner for service users who experience health care difficulties, prioritising workload to meet service priorities and respond to changing needs. To contribute to a nursing assessment within the multi-disciplinary team by providing a nursing perspective and expertise, specialist knowledge, experience and skills.
