Key Responsibilities: Assess and evaluate patients' physical and cognitive abilities to create individualised therapy plans. Design and implement therapeutic interventions aimed at improving patients' comfort and quality of life. Collaborate with the hospice multi- disciplinary team, including nurses and doctors to ensure a holistic approach to patient care. Provide recommendations for assistive devices, home modifications, and adaptive strategies to promote independence. Offer education and emotional support to families and caregivers, helping them understand the role of occupational therapy in end-of-life care. Monitor and document patients progress and adjust treatment plans as needed. Advocate for patient preferences and help manage their care in alignment with their wishes and goals. Work across our In-patient, community, Day Hospice and Wellbeing services. Qualifications & Experience: The successful applicant will have a BSc (Hons) Occupational Therapy or equivalent qualification and be registered with the Health and Care Professions Council (HCPC) Minimum of 4 years of experience working as an Occupational Therapist, with a focus on palliative care, end-of-life, or hospice settings. Proven experience in working with patients facing life-limiting conditions and providing therapy to support functional independence. Excellent communication and interpersonal skills, with the ability to work empathetically with patients, families, and colleagues. Strong problem-solving skills and the ability to adapt interventions to meet individual patient needs. A deep understanding of the challenges and emotional aspects associated with end-of-life care. Desirable: Certification in palliative care or other related advanced training. Experience in a hospice or palliative care setting. Knowledge of local resources available for end-of-life care and family support. Advanced communication skills training
