Crynodeb
Conduct medication reviews (face-to-face, telephone, and via digital platforms) for patients with polypharmacy, long-term conditions, or complex needs. Provide expert advice to clinicians on prescribing and medicines management. Support repeat prescription processes and medicine reconciliation post-discharge. Review and action medication safety alerts and audits in line with current guidance. Undertake structured medication reviews and deprescribing where appropriate. Contribute to QOF, IIF, and other clinical quality improvement work. Provide advice to patients on the safe use of their medicines, side effects, and interactions. Work collaboratively with community pharmacies and local medicines optimisation teams. Promote antimicrobial stewardship and best practice prescribing. Support education and training of staff on prescribing policies and protocols.# Conduct comprehensive assessments of patients to determine their health needs and develop appropriate care plans. Provide advanced clinical care, including diagnosis and management of acute and chronic conditions. Perform physical examinations, interpret diagnostic tests, and prescribe medications as necessary.
