Kingston GP Chambers the GP federation which supports twenty member GP practices across the borough of Kingston are delighted to be supporting Langley Medical Practice in the recruitment of a new Practice Nurse. We are seeking an enthusiastic and motivated part-time Practice Nurse for a minimum of two days per week on Thursdays and Fridays, we are open to consider additional hours if required now or in the future. We are a friendly, supportive and thriving practice looking for a practice nurse to join our excellent team of 3 partners, 4 salaried GPs, 2 practice nurses, health care assistant and phlebotomist. The practice serves nearly 9000 patients from Surbiton Health Centre, a modern purpose built premises, and is five minutes from Surbiton station. The role includes general practice nursing, adult and paediatric immunisations, travel advice and vaccinations, cervical screening, dressings, phlebotomy, long term conditions health management, health checks and the future training opportunity to specialise in Diabetes. Responding effectively to patient need and ensuring patient choice and ease of access to services. Promote healthy lifestyles and participate in national immunisation and screening programmes. Enable patients to manage their long term health care needs and to achieve their full potential in terms of independent living. Actively encourage and facilitate choices for patients by providing health promotion during routine screening procedures e.g: new patient health checks, cervical cytology and long term conditions management. Communicate complex information to patients and relatives where needed with a variety of diverse needs, using negotiation and persuasive skills to motivate. Work with nursing, medical and health care assistant colleagues to ensure the National Service Frameworks are being delivered. Clinical Responsibilities: Undertake clinical duties with relevant training in some or all of the areas The practice operates on a paperless basis. Clinicians are expected to put consultations on the clinical software provided at the practice. Venepuncture when no phlebotomist available. Ear syringing Audiograms Recording of ECGs when requested by a doctor Immunisations for children and adults (both travellers and non-travellers). Complete relevant paper or computer claims for reimbursement /payment by Prescription Pricing Authority and/or Health Authority. Ensure patients that are not entitled to, or receiving, NHS treatment are charged the appropriate fee as advertised in the surgery. Advise individual patients on aspects of their lifestyle to promote good health in accordance with modern practice. Pill check appointments & management of family planning patients. Carry out Health checks on practice patients in accordance with agreed practice protocols. Proactively identify patients with Hypertension and raised Q Risk, code as necessary. Arrange appropriate investigation eg ABPM, interpret results. Arrange investigations for new hypertension patients and follow up. Liaison where appropriate with other professionals and services to provide necessary patient care. Management of patients on chronic disease registers in coordination with other team members and practice protocols. Be involved in clinical audit within the practice team. Offer input, with colleagues, on maintaining up to date practice protocols and procedures; ensuring that wherever possible these reflect best practice. In cooperation with colleagues, maintain treatment room equipment and stock to a safe and satisfactory standard. Stock and pathology ordering in absence of HCA or adhoc as required Adopt the responsibilities of other nurses in their absence, within the limitations of personal professional training and professional accountability. Womens Health Carry our cervical cytology and deal with results in accordance with national and local guidelines. In cooperation with colleagues, strive to maximise the efficiency of the nurse appointments system and develop the services offered by the nursing team to meet the patients and practices need. Administer nebulised therapy to known asthmatic patients. (Known asthmatic patients can be assessed and treated by a Practice Nurse prior to discussion with a GP). Maintaining cold chain with regular audit. Twice daily fridge temperature recording. Maintaining cleaning of treatment rooms and equipment. Attend or complete online training as required by the practice and your nurse governing body keeping your training up to date Arranging investigations for newly raised blood pressures following the practice protocol Additional tasks if respiratory trained: Maintaining monthly recall in view of achieving QOF targets. Offer FeNo and Spirometry testing for diagnosis Maintain log for spirometry testing including calibration, service, cleaning and monthly biological control Other delegated duties considered appropriate to the post. Additional tasks if Diabetes trained*: Carry out 6 month and annual Diabetic health checks using the Diabetes clinical template * For experienced Diabetes nurses opportunity for additional Diabetes role - tbc
