Summary

Key Responsibilities: Conduct comprehensive patient assessments, taking medical histories and performing clinical examinations. Diagnose, manage, and treat acute and long-term conditions, including prescribing where qualified. Work autonomously while collaborating with GPs, nurses, and other healthcare professionals. Provide patient education, health promotion, and preventative care. Undertake clinical audits and contribute to service improvement initiatives. Maintain accurate and timely documentation in line with professional and legal requirements. Participate in training and development programmes to enhance clinical skills. Essential Requirements: For ACP: MSc in Advanced Clinical Practice (or equivalent) and independent prescribing qualification. For tACP: A relevant healthcare qualification (e.g., registered nurse, paramedic, pharmacist, or allied health professional) with a commitment to completing an MSc in Advanced Practice. Current registration with relevant professional body (NMC, HCPC, GPhC). Strong clinical reasoning and decision-making skills. Excellent communication and teamwork abilities. Desirable: Experience in a primary care Ability to work flexibly, including extended hours or weekend shifts if required.
