Community Nurse Section 117 reviewer out of area (Mental Health and Learning Disabilities)

Tri-borough - Hammersmith and Fulham, Kensington & Chelsea and Westminster.

The job role includes the following:
• To be responsible for reviewing Sec 117 aftercare plans, following an agreed process for patients whose responsibility sits with the ICB and are placed out of area.
• To work closely with clients and families, local authority services, placement providers.
• To collaborate on creating crisis and contingency plans.

We are looking for:
• a clinician with experience of or a strong interest in working with people with learning disabilities and mental health challenges
• a clinician who keeps the client at the centre of their practice.
• a strong understanding of the Section 117 aftercare process.
• a dedicated motivated individual who has the experience to work independently with the support of a team.


Could this be you?

The main duties of the job are reviewing the Section 117 aftercare and care plans for clients who reside outside of the tri-borough where responsibility sits with the ICB. You will be working closely with the client, their support network, care providers and local authority.

You will be expected to complete assessments relating to mental health and learning disabilities including identifying strengths and risks and developing aftercare plans reflective of the clients needs.

You will work closely with the ICB lead providing updates on reviews and raising any concerns or positive feedback following a review and support in creating crisis and contingency plans.

We are looking for someone who has experience:
• putting the client at the centre of their practice
• working with people who have a learning disability and/or
• working within mental health
• Understanding of Section 117 after care review process
• Understanding of the Dynamic Support Register
• Knowledge of Positive Behaviour Support

The LD Psychiatry service works across three boroughs offering the psychiatry input into the Tri-Borough LD Community Team. This post offers the flexibility to be based in Hammersmith & Fulham, Kensington & Chelsea or Westminster.

The role requires travel around the UK. We are looking for a clinician who is able to work independently and to organise their time effectively.

You would receive clinical supervision from the Trust Consultant Nurse for Learning Disabilities and be part of the active Learning disabilities and Autism Clinical network. Management supervision would be provided by the LD Psychiatry service manager.

The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.

You will be responsible for reviewing S.117 aftercare plans of Tri-Borough patients who live outside the area. This involves following an agreed review process and creating aftercare plans to support the client.

You will work with the client, family and care services to review Section 117 after care arrangements providing specialist professional advice and expertise in learning disabilities for this cohort of patients.

The post-holder will establish and maintain a high standard of care, ensuring that the quality of care provided is continually informed by the best evidence-based practice.
