Crynodeb
To work as a member of the clinical team of Occupational therapists, physiotherapists and therapy assistants within community therapy services. The post holder will independently manage a caseload that includes some highly complex patients and provide specialist assessments to adults referred to the team who present with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease, within a range of community settings and establishments (including Rehabilitation Wards/Units, Clients Homes, Care Homes and Prisons) On an on-going basis the post holder will assess, diagnose, implement and evaluate individual programmes of care for patients. The post holder will work within a multi-professional team (Consultants, GPs, nursing staff, Occupational Therapists, Social workers and other AHPs) regarding the management and discharge of patients under your care. The post holder will deliver clinical education of all staff as required within community services
