Band 6 Community Nurse - Learning Disabilities (12 months Maternity Cover)
An exciting opportunity has arisen for an experienced Community Learning Disability Nurse to work in the Islington Learning Disabilities Partnership (ILDP).
This is a fixed-term contract for a duration of one year to cover maternity leave. We welcome applications from motivated and experienced Band 5 learning disability nurses seeking career progression into a Band 6 role, as well as those looking to gain valuable experience in a community setting.
ILDP is a forward thinking, fully integrated health and social care community team whose dedicated staff and service providers were awarded the Camden & Islington Team of the Year (2021). We are a well-resourced service and are passionate about delivering high quality support to adults with global learning disabilities and their networks. We are a multidisciplinary team (MDT) consisting of psychiatry, psychology, nursing, occupational therapy, speech & language therapy and physiotherapy and we pride ourselves on our high levels of collaboration, working alongside social work and the Community Support Outreach teams.
Our MDT has reputation for being friendly, inclusive and sociable. We are located in Islington beside the Arsenal and enjoy the benefits of our own building with many of our day provisions and residential services being a short walk away.
Islington Learning Disabilities Partnership aims to provide accessible, non-stigmatising local services that contribute to the assessment, continuing care,rehabilitationand enablement of people with learning disabilities, who may also have physicaland / ormental healthchallenges.The service iswholly committedto the use ofPerson-Centredapproaches, enabling service users to maximise their potential.
The post holder will:
• Be a member of ILDP multi-disciplinary integrated health and social care team.
• Participate in new initiatives to improve the nursing and wider service, such as quality improvement projects.
• Be accountable for a caseloadcomprisingof people with learning disabilities, who may have complex physical and / or mental health needs.
• Act asaCare Coordinator.
• Incorporate health promotion, facilitation,teachingand service development into your role.
• Be aware ofpreviousinitiatives and policies that are now embedded into RNLD practice; such as theLeDeRprogramme and STOMP, etc.
• To complete continuing health care assessments and reviews,as part of the wider MDT.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. To be accountable and responsible for a defined caseload that reflects the priorities of the local population by carrying responsibilityfor the assessment of care needs; the development,implementationand evaluation of programme of care; setting standards of care; and liaison with other agencies, particularly in relation to:
Close working with other services aiming to reduce health inequalities and improve access to mainstream mental health, acute careand primary care services.
Providing an effective ‘in reach’ service to people with learning disabilities who have been admitted to mental health and forensic wards, thereby facilitatingeffective andtimelydischarge.
The provision of early intervention aimed at promoting and maintainingthe physical and mental health of individuals and their families or carers.
To work as a member of the multi-professional team, to attend regular team meetings, supervision forums,participateconstructively in team processes and contribute positively to overall service developments.
To receive referrals as an autonomous practitioner for service users who experience health care difficulties, prioritising workload to meet service priorities and respond to changing needs.
To contribute to a nursing assessment within the multi-disciplinary team by providing a nursing perspective andexpertise, specialist knowledge,experienceand skills.
