Phoenix Care Staff are seeking Support Workers who have a minimum of 6 months experience working in the care sector.

• Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal compliance.
• Develop flexible, engaging activity plans that promote personal growth.
• Support individuals in maintaining healthy lifestyles and managing their health needs.
• Facilitate effective communication and maintain detailed, person-centered records.
• Oversee medication processes and support individuals in self-administration.
• Maintain a safe, clean-living environment and adhere to health and safety protocols.
