Crynodeb
The post holder will be responsible, alongside another, established CNS, for patients with chronic kidney disease within community clinics located in GP practices across Leeds and Mid-Yorkshire. The post-holder will be responsible for the anaemia management of the patients reviewed within these clinics. Going forward, there will be the opportunity for the team to develop a co-ordinated anaemia management service across other renal pathways, including nephrology, pre-dialysis and transplant. The post holder will demonstrate evidence based care in the independent assessment, care planning, treatment and evaluation of a patient group presenting with stable chronic kidney disease, within agreed protocols. This will be facilitated by the Consultant Nephrologists. In addition to the commitment to outpatient review of patients, the post holder will develop an education programme for practice nurses within primary care in these localities. 3. JOB DIMENSIONS The post holder will work within a GP practice environment caring for their own caseload of patients with stable chronic kidney disease. The post holder will also undertake a weekly MDT caseload review with the supervising renal consultants. The post holder will predominantly work independently whilst also being part of an established nurse specialist team.
