Practice nurse team leadership. Lead, support and manage other members of the nursing team, including trainees. Ensure skills of practice nurse team are appropriately and effectively used to deliver care. Support the nursing team in developing their skills and expertise. Ensure an effective nurse appointment system, in collaboration with practice manager. Ensure adequate and appropriate practice nurse cover is available within practice. Provide professional advice on Practice/G DOC developments, recommending changes to working practices in line with evidence-based knowledge. Lead and monitor thedevelopment and implementation of policies, standards and guidelines across the practice nursing team. In collaboration with the Practice Manager and the Recruitment Team, manage the recruitment, onboarding, induction and ongoing employment of the nursing team, including nurses, phlebotomists and healthcare assistants. Working with the Chief Nursing Officer, and Lead GP, develop and monitor key quality markers, thereby promoting clinical effectiveness. Monitor and advise on the effective use of physical and financial resources by the practice nursing team, including ordering. Ensure a robust system for nurse appraisal and revalidation Enable the practice nursing team to apply and improve clinical governance and risk management systems. Primary care nurse duties including (but not limited to) face to face consultations, video consultations, telephone consultations, e-consultations and queries. Clinical administration, and dealing with queries, paperwork and correspondence in a timely fashion. Provide clinical care to registered patients, within your skills and competence, to include normal general practice nursing (immunisations, wound management, minor ailments, aural toilet, health checks, therapeutic monitoring etc) and any areas of clinical expertise, for example long-term condition management. Provision of care for acute and long-term conditions (physical and mental), and contributing to the delivery of QOF and enhanced services. Provision of preventative care, including health promotion advice, vaccinations and screening. Plan, develop, implement, and evaluate new health promotion and long-term condition care. Support of patients in the developing self-management strategies and in fostering patient engagement with the Practice. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Recording clear and contemporaneous I.T. based consultation notes to agreed standards; Collecting data for audit purposes as per Practice protocols. Reviewing patients in the interests of quality improvement and safety. i.e. MRHA safety alerts and dementia reviews. Prescribing in accordance with locally agreed or national guidelines. Cover for other practice nurses will be provided by reciprocal sharing of correspondence, results and other administration. Plan, develop, implement, and evaluate new health promotion and long-term condition care. Contributing to patient safety by maintaining good infection control thorough monitoring and maintenance of stock and equipment to include refrigeration, sterilising equipment and emergency equipment. Complying with the Nursing and Midwifery Councils Code and professional standards for nurses. Complying with CQC requirements for primary care. Complying with the training requirements of G DOC and of the NMC Complying with requirements for revalidation.
