Summary
The States of Guernsey are seeking to appoint a Interface Pharmacist to join the team within the Pharmacy Service.
•The postholder will be responsible for the provision of expert advice on medicines at the healthcare interface by developing strong relationships with stake holders to ensure the safe, cost-effective use of medicines that meets the needs of the patients in the local health economy in accordance with the Partnership of Purpose. •They will be the deprescribing champion for the organisation and will empower hospital and community pharmacists to support deprescribing.
•With particular emphasis on multi-morbidity, polypharmacy and deprescribing, they will design, develop and implement clear joined-up medicines processes across the primary/secondary care/ care homes/other setting interface in order to ensure safe, effective and cost effective use of medicines.
•The postholder will design, develop and implement processes to ensure that accurate information is received in a timely fashion from Primary Care to Secondary care and vice versa when a patient is admitted to hospital or discharged from hospital
