The Bracknell Community Mental Health Team for Adults has an opportunity for a clinician passionate about supporting adults with mental illness and those individuals working with them. We are looking for an Advanced Mental Health Practitioner to join our dynamic multi-disciplinary team operating Monday – Friday during the hours 9:00am – 5:00pm.

The Bracknell Community Mental Health Team is jointly managed and jointly funded by Berkshire Healthcare NHS Foundation Trust and Bracknell Forest Council. The main aim of the team is to provide a range of treatment interventions including medication and psychological therapies to support adults with significant mental illnesses in the community, as well as their carers, and to coordinate the services they receive.

The post would be best suitable for someone with experience of working in an Adult mental health setting. You would need to have significant experience of working with people with mental illness and applying a range of recovery-focused treatment interventions. This role requires the individual to work collaboratively with other disciplines and other teams in the locality to ensure a comprehensive service is provided to our diverse population.
• Providing continued clinical advice, training, and supervision to a group of staff operating in the multi-disciplinary team.
• Liaising with, developing, and maintaining a working relationship with other services in the locality, to ensure the safe and effective transition of clients between different services.
• Directing and supporting the team’s allocation of new patients, including risk assessments, formulation, and recommendations for ongoing care and/or transfer to the most appropriate care pathway.
• Responsibility of own caseload of clients presenting with complex mental health needs.
• As this is a varied role, you will be expected to undertake a range of other responsibilities too.

The “must haves” for this role:

1. Current professional registration with an approved Nursing professional body
2. Leadership or management qualification, or equivalent experience
3. Significant experience of working with adult mental illness and applying a range of interventions
4. Experience and understanding of relevant legislation (i.e. Mental Health Act, Mental Capacity Act, Care Act) and their application in practice
