We are looking to recruit an experienced senior nurse to work as a Band 7 Community Neurology Specialist Nurse with interest in Parkinson’s Disease/MS in a joint role working across Essex Partnership University NHS Trust and Princess Alexandra NHS Trust.

We welcome applications from experienced or existing Band 7 Parkison’s Disease and MS Specialist Nurses as well as nurses working at Band 6 level aspiring to progress into a specialist nursing role within neurology.

The post holder will develop, implement and evaluate a seamless community PD/MS specialist service ensuring that patients receive the highest standard of clinical care and that specialist advice, education and support to health professionals and patients is provided. The post holder will drive service development, audits and research within the Community Neuro service in conjunction with Princess Alexandra NHS Trust.



They will work collaboratively with the established Primary Care Networks, ensuring cohesive leadership across the three localities.



The post holder will be an integral part of the West Essex Care Closer to Home model to meet the needs of the local population . In addition they will work closely with the Virtual Hospital and Care Co-ordination Centre (CCC) to prevent hospital admission, facilitate hospital discharges and ensure care is delivered in the right place at the right time.

· Work jointly with Princess Alexandra NHS Trust Neurology Team to contribute to a seamless service for patients.

· Function autonomously in an adept manner to assess, manage, plan and implement treatment tailored to individual patients across the Parkinson’s disease spectrum in both routine and non-routine circumstances.

· Demonstrate an equitable service through audit and clinical effectiveness

· Maintain the Parkinson’s Disease/MS patient caseload including acceptance of appropriate referrals, triage, waiting times and discharging patients to a dormant caseload when active input is no longer required

· To develop, deliver and evaluate a plan of care relating to symptom management, which will include assessment, planning, implementing, reviewing and documenting care plans in agreement with the person with Parkinson’s Disease/MS. This will include but not exhaustive of ordering diagnostic tests and making referrals.

· Support and facilitate patients to self-manage their disease ensuring they know when to seek help.

· Actively participate in service development.

· Active involvement in a multidisciplinary approach to the decision making process regarding commencing disease modifying treatment. Participate in ongoing delivery of treatment and monitoring of patients while on treatment

· To actively involve service users in providing feedback of their experience of the current service and suggestions for improvements.

ROLE SUMMARY

We are looking to recruit an experienced senior nurse to work as a Band 7 Community Neurology Specialist Nurse with interest in Parkinson’s Disease and MS in a joint role working across Essex Partnership University NHS Trust and Princess Alexandra NHS Trust.

We are seeking a knowledgeable individual with highly developed interpersonal and excellent communication skills and a passion for delivering excellence in care and driving improvements and developments, along with Clinical Research, across the Neurology Service within both NHS Trusts.

We welcome applications from experienced or existing Band 7 Parkison’s Disease Specialist Nurses and MS Nurses as well as nurses working at Band 6 level aspiring to progress into a specialist nursing role within neurology. With training and supervision, the individual will be able to develop the ability to use their clinical expertise and judgement to enable delivery and co-ordination of both routine and complex care to meet the needs of the patients.



They will work collaboratively with the established Primary Care Networks, ensuring cohesive leadership across the three localities.







KEY RESPONSIBILITIES CLINICAL

· Work jointly with Princess Alexandra NHS Trust Neurology Team to contribute to a seamless service for patients

· Function autonomously in an adept manner to assess, manage, plan and implement treatment tailored to individual patients across the Parkinson’s disease spectrum in both routine and non-routine circumstances.

· Demonstrate an equitable service through audit and clinical effectiveness

· Employ a collaborative approach with the wider multidisciplinary team to ensure care for patients and their families is effectively coordinated and their needs are met.

· Continually develop and update knowledge and highly specialist skills to maintain specialist competence and ensure delivery of evidence based care.

· Develop and maintain current knowledge, skills and competence to ensure delivery of evidence based care.

· To be professionally accountable and responsible for all aspects of own work, including the management of own caseload.

· Maintain the Parkinson’s Disease/MS patient caseload including acceptance of appropriate referrals, triage, waiting times and discharging patients to a dormant caseload when active input is no longer required

· To develop, deliver and evaluate a plan of care relating to symptom management, which will include assessment, planning, implementing, reviewing and documenting care plans in agreement with the person with Parkinson’s Disease/MS. This will include but not exhaustive of ordering diagnostic tests and making referrals.

· Design and deliver education and self-management courses to people with PD/MS and their families. Provide specialist education to other professionals involved in the care of people with Parkinson’s Disease/MS

· Provide care in a timely manner in line with local and national protocols and guidance.

· Support and facilitate patients to self-manage their disease ensuring they know when to seek help.

· Have awareness of the resources available and the need to work within the financial envelope.

· Actively participate in service development.

· Active involvement in a multidisciplinary approach to the decision making process regarding commencing disease modifying treatment. Participate in ongoing delivery of treatment and monitoring of patients while on treatment

· Provides and receives highly complex, sensitive and potentially distressing information.

· Overcomes barriers to understanding, such as stress or poor cognition, demonstrating empathy and reassurance.

· Communicates openly and appropriately with patients and carers in situations of highly distressing or sensitive nature, such as discussions with the person with MS and their family around end of life care matters.

· To actively involve service users in providing feedback of their experience of the current service and suggestions for improvements.

· To contribute positively to the leadership within the care group acting as an effective role model.

Organisational



·To work within EPUT Policies, Procedures and Guidelines.



·To ensure accurate and timely reporting of accidents, incidents and near misses.



·To be responsible for signing off incidents using the DATIX system within 10 working days of the incident and following Trust policy and procedures.



·To submit statistical and contractual monitoring returns within agreed timescales.



·To be responsible for ensuring Mandatory and Statutory training is booked and completed within time scales.



·To be responsible for booking and taking annual leave as laid down in Trust policy and Standard Operating Procedures.



·To participate in clinical supervision Professional



·To continuously review practice and to implement change as appropriate and defined within the trust policy



·To work according to the NMC Code of Conduct and professional practice and the relevant legislative framework and exercise professional accountability at all times.



·To be responsible for keeping professionally updated and registered with the NMC.



·If the post holder has a recognised Nurse Prescriber qualification they must prescribe according to agreed protocols and polices.



·To participate in and lead on audit and research as and when required.



·To understand the issues of confidentiality that are inherent within this role and ensure junior members of the team are aware of their level of responsibility in this area.



·To participate in supervision and appraisal process ensuring staff have 1:1 support.



·To provide information to patients, their carers both formal and informal and families so they can make informed choices about current and future care needs.



·To identify health needs in the community and Practice Population and participate in programmes of Health Education and Prevention



·To work in collaboration with the Neighbourhood Teams and statutory and voluntary agencies to provide care for patients and their families and informal carers in their own home, within given resources.



·To prioritise and coordinate the multiple health care needs of the patient.



·In conjunction with the patient/carer to develop a personalised care plan that reflects individual needs, preferences and choices and provide a Person Held Record for those patients with complex needs Communication



·To establish and maintain effective communication and liaison with the Neighbourhood Teams and other agencies involved in caring for patients i.e. social care, acute trusts, voluntary and statutory agencies, relatives and informal carers. This process will require coordination within the individual primary health care teams including the development of regular meetings



·To effectively communicate at all levels of the organisation; to a variety of healthand socialcare professionals working in primary, community and secondary care, out of hours providers, voluntary organisations and patients and carers



·To maintain accurate contemporaneous and up-to-date patient records and inform other appropriate professionals about changes in conditions.



·To provide high quality written reports and any other written documentation as necessary.



·To participate in and lead on any of the professional groups set up for the development of policies, protocols, standards, audit and development of the Community Nursing service



·To lead on clinical audit within practice in order to determine standards of practice and use research awareness skills to critically appraise the effectiveness of practice



·To provide support, education opportunities and training for other staff and students including pre- and post- registration nursing students



·To access appropriate and relevant training opportunities that will enhance both personal and professional development.



·To ensure team members have accessed appropriate training to effectively manage the patient caseload Leadership and Management



·To exercise leadership and judgment in the management of a Community Nursing Team to ensure good clinical practice and maintain high standards of care, and review practice at regular intervals.



·To manage a delegated budget where appropriate



·To manage the performance of staff and identify training and development needs of staff and students.



·To participate in the Trust appraisal process



·To ensure all team members work according to Trust policies and procedures.



·To participate in the recruitment and selection process where appropriate



·To effectively manage staff who do not meet the requirements as set out in their job description



·To liaise with other Team Leaders to ensure equity of staffing and work load levels to meet the needs of the service



·To effectively manage all staff within the teams including HCA’s and registered nurses.



ADDITIONAL DUTIES

In addition to the above duties you will also be expected to perform the below key activities in line with your job role;

- Complete mandatory training in line with Trust policy and procedures

- To participate in the staff appraisal process and to undertake for any staff you manage

- To keep yourself updated on all matters relating to Trust policy

- To provide management supervision where appropriate

-You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services

- To be able to travel to different Trust locations within the West Essex (Uttlesford, Harlow & Epping) area to support services when required
