Summary
The post holder will be based within the Social Care Hub and will work as part of a centralised Social Work Team. Within the Social Care Hub, social workers have a specific focus upon the needs of adults of working age with mental health problems who have social care needs. They will lead on: Care Act Assessments Requests for Tribunal Reports for unknown cases from Wotton Lawn/Charlton Lane/Recovery Units and people who are from out of county Reviews of individuals that Gloucestershire have placed out of county Reviews of accommodation cases Section 42 enquiries Provide a multi-organisational crisis contingency and relapse planning function Be the link for those that Gloucestershire has a Section 117 responsibility for, but reside outside Gloucestershire Undertake the interface social care transition work for young people moving into adult services. Act as Practice Educator for Social Work students and any ASYE social workers
