Crynodeb
The post holder will work as part of the integrated Multidisciplinary team under the supervision of the Clinical Team Manager. He / She will be accountable for the delivery of high quality care to patients within their own homes. Working with other members of the IMT to provide care to and promote health/well being and independence for patients on the team caseload, working closely with other partners/stakeholders including GPs, Tissue Viability and vascular clinics.
