The Substance Misuse Service (SMS) provides clinical interventions for prisoners with 'Substance Misuse' health needs in HMP Aylesbury. The post holder will work as a member of the SMS team contribute to; deliver high quality, safe and effective Non-Medical Prescribing Services.

The post holder will be responsible for the delivery and monitoring of high standards of substance misuse nursing care to patients assessed and admitted to the ISMS. This will be done in accordance with their clinical needs applying evidence-based practice to achieve positive clinical outcomes. The post holder will adhere to relevant clinical protocols and policies. The post holder needs to be thoroughly familiar with all aspects of security and safety consistent with working within a custodial setting.

· assessment, planning; implementation and evaluation of the care plan for patients referred to this specialist service.

· advice on treatment options including the full array of substitute prescribing, the management of alcohol withdrawals, detoxification, titration and symptomatic relief.

· dispensing of medication including controlled drugs, and managing all aspects of clinical safety e.g. potential overdose, pertinent to a high-risk environment.

To participate and lead in multidisciplinary team meetings/case discussion.

· To be a member of the, SMS team meetings, 'Medicines Management' group and attend any business or clinical meetings delegated by the MH Lead.

· To ensure that regular handover meetings take place, and that all information is communicated across the team. Use the electronic record system to communicate with team members and other healthcare services throughout the prison to ensure auditable communication.

· To provide specialist advice and support to colleagues throughout the prison
