Crynodeb
Act as a point of contact and co-ordinate admission to inpatient services. Act as chair for Bed Management meetings as required and maintain and share accurate records. Act as a chair for daily board rounds Liaise closely with Clinical Leads, Service Managers and Ward Manager re bed usage, barriers to discharge escalating areas of concern as appropriate. Attend MDT Ward Reviews to track service users progress, identify barriers to discharge and facilitate discharge as clinically indicated Work closely with Community Multi-disciplinary teams. Liaise closely with other hospitals and service providers regarding bed management issues. To carry a contact telephone during working hours and always be contactable, updating the wider Patient Flow Service throughout the shift To contribute to reviews of bed management procedure / policies. Provide written reports as necessary including but not limited to reports on delayed discharges, readmissions and out of area placements. Provide information to support responses to the ICB and others as is required. Be able to communicate effectively and fluently to other professionals and signpost where necessary.
