An exciting opportunity to join Chesterfield Royal Hospital Maternity Services as a Lead Nurse for Transitional Care.

We are currently implementing Transitional care on our postnatal ward. The lead role will facilitate the implementation / management and evaluation of the Transitional Care service, working alongside the Maternity and Neonatal teams.

The unit will care for infants who meet the criteria for Neonatal Transitional Care as described in CNST Safety Action 3. The beds are currently part of the bed base on the maternity ward at Chesterfield Royal Hospital. Our team provides high quality care, working collaboratively to keep mothers and babies together, avoiding separation and admission to the neonatal care unit.

Our maternity inpatient ward offers a calm and welcoming environment for women and their families, whilst Chesterfield Birth Centre provides co-located midwife and consultant led labour ward facilities, including 4 birthing pools

We are a fully accredited Baby Friendly unit.

Our Community Midwifery service centre's around 4 local bases /GP surgeries which provide care for around 2500 women per year.



The overriding purpose is to support the provision of the highest quality patient care through personal actions and continuous improvement.

To work as the Lead Nurse, to lead and work alongside the Ward Leader in providing high standards of holistic transitional care, ensuring that it is evidence based and compliant with best practice recommendations for Transitional Care services (TC).
