About the Role:
We are seeking an experienced and dedicated Occupational Therapist to join our Contact Team. This role offers a dynamic and fast-paced environment, where you will handle a high volume of referrals and manage straightforward OT cases. The successful candidate will be required to work two days per week based at our office.
As an Occupational Therapist within this team, you will be responsible for screening and assessing referrals, providing professional advice, and ensuring that the right services are directed to clients. You will collaborate with other healthcare professionals and departments to ensure efficient and effective delivery of services. Additionally, you will be required to complete occasional home visits to assess the needs of clients in their own environment.
Key Responsibilities:
Screen and assess referrals: Review and manage incoming referrals, ensuring the appropriate allocation of cases to the right OT resources.
Provide clinical advice: Offer expertise in OT assessments for straightforward cases, ensuring timely and effective interventions.
Manage caseloads efficiently: Prioritize and manage a high volume of referrals while maintaining quality standards of service.
Collaborate with other teams: Work closely with multidisciplinary teams, including social workers, healthcare providers, and other relevant professionals, to ensure comprehensive care for clients.
Complete occasional home visits: Conduct home visits as necessary to assess the client’s living environment and ensure recommendations are appropriate for their needs.
Ensure accurate documentation: Maintain clear and accurate records of assessments, interventions, and recommendations in line with local policies and procedures.
Client-focused approach: Always aim to provide the best possible outcomes for clients, ensuring their functional needs are met effectively and appropriately.
Required Qualifications and Experience:
Qualified Occupational Therapist with a valid HCPC (Health and Care Professions Council) registration.
Experience in Occupational Therapy: At least 2 years of experience working as an Occupational Therapist, with a strong background in handling high volumes of referrals and managing straightforward cases.
Strong communication skills: Able to engage with clients, families, and other professionals effectively, both verbally and in writing.
Ability to work under pressure: Manage multiple referrals and meet deadlines without compromising quality of care.
Proficient in case management and report writing: Experience using OT assessment tools and ensuring thorough documentation.
Team player: Ability to collaborate with colleagues and professionals across different settings.
Good organizational skills: Efficient in managing a caseload, prioritizing tasks, and maintaining a structured approach to work.
Ability to conduct home visits: Comfort and competence in conducting occasional home visits as part of the assessment and care planning process.
Desirable Skills:
Experience working in community settings or within social care services.
Knowledge of OT interventions for older adults or individuals with mobility challenges.
Experience with referral triage and effective prioritization of cases.
Proficiency in using OT-related software and databases
