We are seeking a compassionate and dedicatedCAMHS Transition Family Support Workerto join our dynamic MK Community Mental Health hub. You will work under the supervision and direction of our CAMHS Transition Practitioner, with the opportunity to work autonomously across a range of clinical and community settings.

Join us and help make a real difference in the lives of young people with complex mental health needs and their families.
• - Support qualified staff with joint visits, organising and attending meetings, case reviews, chronologies, assessments, and related documentation
• - Make referrals to external agencies, contributing to the management of a shared caseload
• - Provide practical support and therapeutic interventions to children and young people with complex mental health needs and their families/carers
• - Work closely with parents/carers to offer support in collaboration with key workers, other Trust staff, and relevant external agencies
• - Participate in a multi-disciplinary team to contribute to holistic care plans for children and families
