An exciting and challenging opportunity has become available for a Band 3 Triage Practitioner.

The post holder will be responsible for ensuring immediate care to the relevant patient group and will work within the pre-operative assessment and admission service.

We are a busy department assessing all patients undergoing surgery, many with multiple co-morbidities.

You will work closely with a wide MDT, including other hospitals and GP services to ensure a smooth, safe and positive journey for all our patients.



The Post Holder is responsible to deliver competent, safe and systematic care to patients that are referred to Pre-Op Assessment clinic.

The post holder will be able to perform a variety of duties including swabbing, deliver basic care needs, phlebotomy, perform ECG, assist with general anaesthetic day case triage and be able to follow protocols and Standard operating procedures to ensure a seamless patient journey.

Acute Care

Providing emergency and maternity services at Whiston, Southport and Ormskirk hospitals, and medical and surgical specialties across all our sites.

Providing primary care services at Marshalls Cross Medical Centre situated in St Helens Hospital.

Community Services

Providing adult community services for St Helens and a wheelchair service in Chorley, South Ribble, and West Lancashire. Our inpatient unit at Newton Community Hospital is where patients needing acute hospital beds can continue rehabilitation, freeing up space for more unwell patients. We also provide urgent care at our Urgent Treatment Centre located in St Helens town centre.

Specialist Regional Services

We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.

KEY DUTIES

To ensure that privacy and dignity of patients is paramount and is not compromised.

To ensure that all patient information is confidential and is not disclosed to a third party except with permission of the Nurse in Charge.

To report any untoward incidents/occurrences via the datix system

To ensure that all patient care is delivered with the highest degree of professionalism.

CLINICAL & PROFESSIONAL RESPONSIBILITIES

To be able to provide care and emotional support to patients and their relatives who are planned admissions for general Anaesthetics surgical procedures.

To participate in the holistic pre-operative assessment of patients, to recognize and organize individual patient care needs and to be able to manage own workload and prioritize care.

Make appropriate observations of physical, emotional and psychological state of an individual and report any abnormalities to the relevant practitioner. Complete relevant documentation as per protocol.

To develop own knowledge/practice and have willingness to learn new skills.

To contribute to the setting, maintaining and monitoring of pre-operative assessment modernization standards within the unit.

To have responsibility for the security and maintenance of equipment belonging to the unit.

To demonstrate awareness of Infection control policies and procedures

ADMINISTRATIVE RESPONSIBILITIES

Ensure patient records are kept up to date and accurate and recording is done in line with Trust policies and procedures.

TEACHING & TRAINING RESPONSBILITIES

To attend and manage own competencies, actively ensure that you are up to date with e-Learning, on line training and bar code training

To participate in an annual performance review undertaken with the Manager to identify personal development needs.

To participate in the teaching of bank staff and colleagues through the in-service training of health care assistants, and to participate in the induction of new starters.

RESEARCH & AUDIT

To commence and participate in research/special projects/ audit as directed by the team lead.
