Crynodeb

To demonstrate proactive and complex communication strategies to meet the needs of patients, relatives, and colleagues where there could be barriers to understanding. Liaise with nursing and non-nursing colleagues and offer advice, guidance, motivation, and counsel as appropriate. Anticipate patient's reaction and plans appropriate style in view of patient problems and lifestyle. Use knowledge and underpinning skills to interpret measurements and act on observations in clinical situations in collaboration with nursing staff and medical staff with the urgency that the individual situation requires and follows through to a satisfactory conclusion. Participate in senior leadership group, clinical governance, and any other Trust wide meetings. Assess and diagnose a variety of patients presenting with undifferentiated minor injuries/minor illness conditions for both adults and paediatrics as appropriate. The process will often be independent of medical colleagues. Utilise skills of clinical history taking / physical examination in order to formulate / test hypothesis and reach differential diagnosis. Use enhanced examination skills of all relevant bodily systems e.g. Cardiac / Respiratory / Neurological / MSK etc. To make clinical judgements as to the best course of action or treatment, accepting risk for those decisions that do not led to further patient assessment. Appropriately request and interpret x-rays in line with Trust policy. Plan, implement and evaluate treatment / interventions and care for patients presenting with an undifferentiated diagnosis. Provide or recommend treatment, discharge or refer people with any clinical condition within your scope of practice, using clinical judgement and skills according to differential diagnosis and identified / negotiated patient needs.
