Refer to Job Description Key Relationships: The post holder will be required to communicate and provide highly complex information to a wide range of internal and external stakeholders, including service users, wider public, public health, specialist commissioning, Local Authority / Education / Strategic Commissioning Team, NHS England and clinical networks to ensure a collaborative joined up approach to commissioning e.g. across the universal and targeted pathways, for those with Continuing Healthcare needs. Nurture key relationships and maintain networks internally and externally, including national networks. Key relationships will include Chairs and senior staff in the Integrated Care Board and Senior Clinicians in all settings, the Director of Childrens Services, and, Local Authority Elected Members. Develop relationships with individual service users and service user engagement forums to ensure CHC eligible individuals are involved in coproduction and management of individual packages of care. Implementing and monitoring the use of NHS Englands NHS Continuing Healthcare Competency Framework, to help ensure that all staff within the service are competent to deliver the requirements of their roles. Sourcing appropriate training to meet the needs of the service Using feedback from individuals being assessed for or receiving services, or from their representatives, to inform training. Ensuring any learning from incidents, case reviews and complaints is used to improve practice, and implementing any changes to training as required. Carrying out audits and supporting others to carry out audits, using those audits to bring about service improvement, inform training needs and /or policy refresh. Present highly complex information about projects, initiatives and services to a wide range of stakeholders in a formal setting.
