JOB DESCRIPTION Post Title: Specialist Midwife Diabetes Term of contract (if appropriate): Permanent Pay Band: 7 Hours: 37.5 Location / Department: Maternity/Antenatal Clinics at AGH, KTC and WRH Accountable to: Director of Midwifery Reports to: Maternity Matrons for Antenatal Services Key Working Relationships: Internal Antenatal Clinic Managers and staff Medical Staff Midwifery and Nursing Staff Risk and Governance Team Maternity Matrons External Women and their families Public Health England LMNS Relevant external partners Liaises with: Maternity Matrons Job Purpose: Provide professional leadership and act as lead Midwife on diabetes within the Maternity service and act as a positive role model To assess and provide the full range of care needs to develop, implement and evaluate programmes of care throughout the antenatal, intrapartum and postnatal period for women and their babies. Develop, deliver, implement, evaluate, audit and adapt individualised plans of treatment ensuring that appropriate care pathways are adhered to. Recognise the need for referral to other agencies and/or services as required. To supervise junior staff and to teach qualified and unqualified staff, acting as a mentor, as appropriate, providing advice and support as necessary. Work autonomously within guidelines and spheres of professional practice. Ensure a high standard of care for diabetics in all areas of the service, encouraging them to manage their condition and reduce their length of stay where possible. Evaluate local services against national standards. Assist Midwifery managers with clinical governance action plans Comply with statutory requirements of NMC registration Key Duties: This will be both a strategic and clinical role. The successful applicant will work with service users (including the MVP), the multi-disciplinary team and local stakeholders to advance and protect health and wellbeing of those in our care, and improve health outcomes of the poorest families (Department of Health 2013). The successful applicant will develop and implement processes to evaluate and improve patient experience. The post holder will possess the necessary skills and experience to liaise with both our inpatient services and affiliated external partners to coordinate, standardise and streamline our service provision across the county. You will be required to represent WAHT at local, regional and national level when appropriate, planning and influencing service development in relation to public health. Clinical Provide visible leadership and accessible support to the Midwifery and Multidisciplinary Teams Act as a professional role model Obtain and disseminate specialist advice on relevant Diabetes Matters to all health care professionals providing support and services within the hospital and community settings Take professional and managerial responsibility for the effective delivery of care for women around Diabetes in accordance with national, regional and local guidelines Develop, inform and implement strategies to ensure health professionals are adequately trained to support women and their families with health improvement measures pertaining to Diabetes Promote the highest standards of evidence based and audited clinical care Actively encourage service user feedback Lead and guide the development and facilitation of parent Provide expert clinical advice within midwifery services, working closely with the Divisional Director of Midwifery, Matrons and Clinical Director to further develop care for mothers and their families in relation to public health objectives Manage and monitor Midwifery practice in Diabetes care and work closely with the other midwives, the Senior Midwives to ensure protocols, guidelines, policies and procedures are adhered to. Implement changes recommended by latest evidence Escalate and report incidents any issues which may have a negative impact on the provision of the service by adhering to the relevant policies Disseminate contentious, complex and sensitive information appropriately Promote integrated working across the service, liaising with the multi-disciplinary team to promote the service and collaborative working relationships, thus improving the experience of those we care for Maintain own clinical competency in respect of health promotion and wider midwifery practice, utilising opportunities for professional development. Perform comprehensive assessment of women, families and carers, where needs are complex. T. To advise on insulin dosages and regimens for women admitted with poor diabetes control and support with self-management Enables women and families choice and involvement and initiate appropriate action, acting as an advocate for the woman and her family. Co-ordinates care pathway and liaises across organisational boundaries Ensures each woman is placed on the correct treatment pathway at the appropriate time and necessary investigations and treatments are organised Monitors womens compliance against treatment plans through interpretation from test results and clinical findings Considers a range of options and appraises each one, when making decisions about changing care treatments. Takes remedial action if appropriate, based on knowledge and skills to adjust care packages Refers to other disciplines/professions unsupervised. Accept referrals from other disciplines/professionals unsupervised Participates in case conferences in relation to future management or discharge arrangements Reviewing the educational needs of diabetic women admitted with diabetic ketoacidosis/severe hypoglycaemia to determine causes and future prevention. To maintain a visible and predictable input to all areas. To work together with the Diabetes team to reduce perioperative delays in diabetes management. To ensure appropriate follow up in the Diabetes Service for all women converted to insulin Professional Plan, organise and deliver in house training and education, through a variety of formats that meet the needs of different health professionals working with mothers and babies. Maintain an awareness of professional developments in Midwifery and public health outcomes to facilitate the maintenance of appropriate evidence based care Liaise with the multidisciplinary / multiagency team referring to and accepting referrals from medical staff and other health care professionals, internal and external, as and when appropriate Participate in orientation of new staff and students Work within the NMC Code of professional practice, Midwives rules and standards Respect womens equality, taking into account diversity and rights, ensuring that vulnerable women and families receive equitable care including those who are subject to domestic abuse, homelessness and/or language barriers. Act as an innovative and enthusiastic role model; promote an open, honest and transparent culture demonstrating excellent leadership skills Provide and maintain an environment in which individuals can develop their skills and knowledge to their full potential. Provide excellent support for students on clinical placements Support staff in actively participating in personal development through mentorship, preceptorship, mandatory training and appraisal Maintain and promote an environment conducive to learning. Link with networks beyond maternity services to share good practice and information Management and Leadership To work collaboratively with Clinical Audit department to collect, analyse and report on statistical information in line with Care Group, Trust and National requirements. Ensure that high standards of care in relation to public health are provided and maintained and act when standards are not being maintained Be proactive in the development of practice/ services within area of expertise, through business planning process / service development, in conjunction with Senior Managers. The post holder will participate in the on-call management for Women and Childrens on a rota basis. Communicate effectively, throughout the multidisciplinary team and liaising with outside agencies, especially where the womens and families ability to do so is compromised by acting as the patients advocate. Action where appropriate clinical incident reports, complaints and monitoring of risks Formulate, review and implement clinical policies and guidelines within the area of responsibility. Promote best practice in relation to clinical risk and governance issues and identify areas for improvement Provide regular, clear written reports on project progress Work closely with existing diabetes, obstetric, midwifery and anaesthetic colleagues to ensure that communication links are clear and effective To work in accordance with Trust, Directorate and Divisional management guidelines and develop these where necessary Education, Professional Development and Training Participate in regular midwifery and clinical supervision Attend all Trust, midwifery and specialism related mandatory training To take every reasonable opportunity for maintaining, developing and acquiring competencies and skills for self-development Comply with all Trust policies, procedures and protocols Carry out duties with due regard to the Trusts Equal Opportunity Policy.
