Following promotion, we are recruiting to our Head of complex care post. This role is open to candidates who hold a professional registration and has significant clinical experience working in a variety of mental health settings. Expertise in working with people with complex care needs is essential and a demonstrable ability to work in partnership with stakeholders. This role is particularly suited to clinicians with a vast experiencein supporting people to improve occupational performance and increase social inclusion.
The successful candidate will oversee the assessment processes for people with complex care needs, lead on the identification and review of community health funded accommodation placements and contribute to a trust wide mental health rehabilitation offer. The ability to assess function, and understand all factors impacting on performance, is vital. This role demands significant attention to detail, an advanced ability to build and maintain relationships as well as substantial experience in having challenging conversations.
There is a need to travel to various locations within, and outside the borough. Working across various team bases are beneficial to help build and maintain the vast amount of relationships within this role.
This role is line managed by the Directorate Lead for Occupational Therapy, with additional support from the Borough Director and Associate Clinical Director for Inpatient services.
-To lead coordination of complex care health funded placements
-To oversee the assessment process for people with complex needs, identifying the right type of placement, managing placements including adherence to care plans and reviewing progress, as well as supporting good transitions between placements.
-To lead the process by which we support move-on / step down to community placements for people with complex needs admitted to acute wards, or who reside in existing community placements.
-To keep an overview of all ELFT funded placements.
-To offer staff training to upskill workforce in the support to people with complex needs
