The Manchester Care Group have developed a multi-disciplinary hospital based integrated discharge team to support the safe and rapid discharge of service users who no longer require a hospital setting. The remit of the service will focus onservice users from all adult acute, later life and PICU beds across the GMMH Manchester care group footprint, contracted beds within the North West Bed Bureau (NWBB) and out of area placements (OAPs).



The integrated discharge team will ensure the facilitation of timely discharge from the inpatient units across the locality and wider GMMH foot print, across the NWBB and OAPs to reduce wasted bed days in a hospital setting for Manchester locality patients. The post holder will lead on facilitating the discharge of complex cases and develop excellent working relationships with external partners to progress barriers in relation to service users deemed clinically ready for discharge.





The successful candidate will be required to undertake specialist mental health assessments as required andprovide bespoke interventions for identified service users within inpatient services with the aim of engaging people in the most appropriate service for their needs to reduce re presentations. / Inpatient admissions.

They will offer dual assessment, formulation, brief interventions and work to support the development of joint working protocols between mental health services and substance misuse services. This will occur either during the inpatient stay or as part of their discharge plan and handover to community mental health and/or substance misuse services or supported accommodation and the role will be pivotal in supporting discharge planning.



The successful candidate will be required to develop a good working relationship with ward staff and across the multidisciplinary team. They will develop and promote their own role within the service in which they are based and encourage staff to refer patients.



Central to the care approach in this service is a focus on ensuring that people are not excluded from mental health services because of cooccurring substance misuse nor from substance misuse services because of cooccurring severe mental illness. This post covers multiple inpatient sites and ability to travel is required.

· To develop specialised formulations and advice for referred clients

· Toensureinformationaboutservicesandpathwaysiscurrent and accessible and promote positivity within the wider health and social care system.

· To advise on specialised risk assessment and risk managementofclientspresentingwithvulnerability,self-harm and/or risk of physical, sexual or emotional harm to others or self in the context of their substance misuse.

· Toprovidegeneraladvicetootherprofessionsonsubstance misuse aspects of risk assessment and management.

· To have an understanding of discharge pathways and processes to support timely discharge and access to appropriate support services in the community

Toeffectivelymanagelevelsofemotionaldistress including expressions of anger, suicide and self-harm displayed by clients.
