Crynodeb
The Royal Orthopaedic Community Scheme (ROCS) provides and early discharge service for patients following elective orthopaedic surgery.
The post holder will work alongside registered nursing and physiotherapy staff to provide post-operative care within the patient's home. You will be required to work with colleagues to provide a service seven days a week. You will be expected to work within the Royal Orthopaedic Hospital and the patient's own home in order to assist in providing a seamless interface between hospital and community, for all patients discharge onto the scheme.
You will be required to visit patient's in their own homes following referral to ROCS. Whilst provide high-quality patient care and advice. This could include wound dressings application and removal, removal of clips/sutures, taking bloods, education of safe mobilisation and physiotherapy exercises, Resolution of any patient concerns with appropriate escalation to senior staff should complications arise. Patient social and physical assessments.
This role will include basic nursing and physiotherapy skills as per competency framework, working as an independent, self-directed practitioner under the in-direct supervision of the ROCS Manager.
You will be working alongside ROCS colleagues as part of an interdisciplinary team to provide comprehensive, seamless, high quality care whilst the patient in on the scheme to maintain safe and supportive care.
You will be expected to maintain accurate, legible, and concise documentation of clinical records.
You will need effective communication skills when liaising with patient's, carer's, GP's and the Nursing and Medical staff at the Royal Orthopaedic Hospital. Correctly following all ROCS policies and procedures.
Working within the requirements of the Health & Safety at Work Act 1974.
