Crynodeb
To work independently managing own caseload in conjunction with the General Practice team. Working alongside the Care Co-ordinator organising and attending monthly MDT meetings. To work closely with multiagency teams to assist in the management of risk, facilitation of complex case reviews and the immediate crisis management, ensuring patients receive appropriate investigation, intervention and care planning to ensure their physical and mental health is optimally and safely managed. To give patients the best possible care for rehabilitation, reablement, recovery or in some cases appropriate palliative care.
