Crynodeb
This post carries continuing responsibility for pro-active management of a caseload consisting of identifiedhigh intensity users with complex needs. The post holder will work autonomously to provide a comprehensive,advanced assessment and care package to the patients and families, utilizing support services and liaisingwith other agencies to ensure the complex needs of this client group are met.To provide advanced nursing assessment, diagnostic, prescribing, care and treatment, casemanagement and anticipatory care for patients with complex LTCs, marshalling resources, overseeingthe quality of care from all agencies and co-coordinating acute, primary and social care ensuring that thefocus of care remains at home and in the community where possible. Word versions of documents are available if needed to allow for any adaptations to be made i.e. font, colour, background, convert to Read Aloud.
