Crynodeb
To provide expertise in diabetes care by providing information, support, advice and specialist knowledge to patients, their families, and carers, staff and to act as a resource to the multi-professional team. To participate in the co-ordination of patient care to ensure continuity throughout the patients care pathway. To contribute to review and development of the service, and to reflect the needs of the patient group. To liaise closely with the health care teams of inpatient wards. To demonstrate knowledge and experience of diabetes management and clinical leadership skills. To support the delivery of a high quality, safe and compassionate healthcare service, all staff are expected to act as a role model to others in all aspects of their work and consistently demonstrate MFT values.
