Roles and responsibilitiesClinical Uses highly developed clinical knowledge to independently assess, plan,implement and evaluate treatments and interventions for patients presentingto the Rheumatology clinic with Axial Spondyloarthritis. Undertakes physical examination as indicated by the patients condition; autonomously analysing complex clinical signs. Request investigations eg blood tests, ultrasound scans and guided injections, X-Rays and MRI scans in accordance with IR(ME)R regulations and local non-medical policies. Analyses multiple sources of data including patient history, physicalexamination and investigation findings when making diagnoses, clinicaljudgements and evaluating care provided; presenting this information tospecialist /senior medical staff for advice if scope of practice is reached Continuously revaluates findings and clinical response to treatment andestablishes an appropriate ongoing management plan accordingly. During patient consultation discusses medication for therapeutic effectivenessappropriate to patient need within the roles scope of practice and in liaisonwith the medical team and prescribing pharmacist. Assesses patients with mental health needs using local policy and guidelinesand refers to the appropriate services. Communicates effectively with patients, carers, colleagues and others usingappropriate communication styles. Anticipates barriers to communication andensures patients and significant others are kept fully informed and consent totreatment. Acts as a resource for staff, advising on local, national and Trust policy,procedures and guidelines ensuring patient safety and clinical governance. Be professionally and legally responsible for all aspects of your work, includingthe management and prioritisation of clinical caseload working as anautonomous advanced physiotherapy practitioner to ensure provision of anefficient and effective Rheumatology service. To provide comprehensive written records of assessment and treatment inmedical notes and physiotherapy notes according to trust standards,including providing a high standard report/clinic letter as indicated. To act as a source of expertise and point of contact for the MSK physiotherapy team in the management and treatment of patients with Rheumatologicaldiagnoses and maintain a clinical physiotherapy caseload.Professional To comply with the physiotherapy codes of professional conduct at all times To be professionally and legally responsible and accountable for all aspects ofthe practitioners professional activities. To be responsible, with the divisional therapy lead, for leadership inmaintaining and improving all aspects of clinical governance, evidence-basedpractice and quality assurance in MSK/Rheumatology practice To be responsible for maintenance of accurate and evaluative written records,to include comprehensive and timely progress reports, clinic letters and referrals. To be responsible for own personal professional development and keepabreast of new advances in musculoskeletal and Rheumatology clinical practice and also support and oversee physiotherapy staff learning. To participate in the appraisal system as both appraisee and appraiser to morejunior staff using trust documentation. To contribute to clinical teaching and staff development of MSK peers,medical students and other health professionals within the Rheumatologyteam. To effectively manage complaints and conflict scenarios, both withinspecialist clinics and MSK physiotherapy department. To understand and comply with all mandatory training requirements andensure your team are compliant with training.Organisational To be responsible for providing an efficient and effective physiotherapy service,by demonstrating good leadership, organisational, time management andprioritisation skills. To teach, supervise and appraise therapy staff within the MSK Physiotherapyteam and assist the therapy lead with any complaints / formal HR processes. To manage and prioritise own workload by appropriately balancing clinical andother demands to ensure all service needs are met. To formulate flexible strategies for clinical education and service developmentwithin area of physiotherapy expertise. To assist with management of the MSK physiotherapy service, alongside MSKcolleagues including patient care, training, supervision, various meetings egmanagement and strategy planning and meetings with other healthprofessionals. To use excellent prioritisation and time management skills to meet theunpredictable and conflicting needs of the service. To work closely with consultants, GPs, service managers and other healthprofessionals to provide a seamless service. To maintain accurate and up-to-date patient records in line with departmental,professional and legal standards, and ensure that all members of your team dolikewise. To be aware of and implement all organisational, departmental and Trustpolicies and to be involved in updating any as appropriate. To ensure timely communication of assessment and treatment findings toother involved disciplines such as discharge and referral letters and ensurethat all members of your team do likewise. To assist in the operational management of MSK physiotherapy. Anticipates potential barriers to communication. Communicates effectively; often about complex, sensitive and potentiallydistressing information with patients, family and/or carers. Creates a trusting partnership with patients and/or relatives/carers in order tocommunicate and explain complex medical issues (including new diagnoses) andagree a management /treatment plan. Ensures awareness of sources of support and guidance such as PALS andprovide information in an acceptable format to all patients recognising andreferring any difficulties and referring where appropriate. Communicates clearly and effectively with colleagues; often receiving or givingcomplicated plans or information of a sensitive nature. Maintains the responsibility for the delivery of a detailed, accurate andpotentially complex clinical handover to colleagues in other specialities andprimary care e.g. GPs. Produces detailed and accurate writteninformation within the medical notes regarding all clinical assessments,investigations completed and requested, and treatments administered Maintains confidentiality as required by professional, local and national policy.Acknowledges situations where there can be a breach of confidentiality. Recognises and defuses potentially aggressive and violent individuals/situationsin line with local policy and legal frameworks.Education & Research Takes responsibility for their own learning and performance including theparticipation in clinical supervision and maintaining an awareness of relevantresearch evidence. Develops health promotion and education in conjunction with other health careprofessionals ensuring that all patient care within the multi-disciplinary team isbased on research and best practice. Acts as a constant source of clinical and theoretical knowledge for all gradesand disciplines of staff as well as patients and their significant others, providingsupport and clinical advice in specialist areas, based on evidenced basedresearch. Possesses proficient typing, IT and computer skills. Uses audio/visualequipment to record and impart information and research. Prepares and delivers presentations and participates in workshops on a local /regional / national level as required to ensure the communication of goodpractice. Works with the MDT to further develop appropriate clinical pathways and careapproaches. Disseminates their own learning and good practice to other teammembers. Designs, coordinates and undertakes collaborative audit, research anddevelopment into the specialty and related practices. Plans, delivers and implements programmes of education and training fortrainee APPs, nurses, medical and other disciplines and continues to beinvolved in their ongoing support.
