Complex Psychosis Practitioner Pilot in CMHT The work in the pilot phase will require a flexible and assertive approach to connect with these service users, as well as developing positive working relationships within the CMHT and AOT. The interventions will be flexible and person centered, with the emphasis on supporting complexities and re-engaging with mental health services. The pilot also has a function of supporting transitions and communication between AOT and the CMHT. This includes acting in a bridging role between both teams to support referrals and transitions, as well as providing advice and consultation around complex psychosis within the CMHT.The pilot role will include attending regular MDT and team meetings with AOT as well as the clinical work within the CMHT. You will be involved in the evaluation of the pilot role along with the Occupational Therapy Rehab Lead for Complex Psychosis. Case Manager in The Assertive Outreach Team To work as a member of the Assertive Outreach team in providing a city wide service for patients with highly complex, severe and persistent mental health problems seven days per week; To provide day to day co-ordination of the team, as required; To act as a case manager, responsible for the management of a case load including the assessment of care needs, the development, implementation and evaluation of care packages; To ensure an effective learning environment for students and other staff, regardless of the professional background; To carry continuing responsibility for the setting and monitoring of standards of care in line with Clinical Governance requirements; To promote nursing values and interventions in the MDT and with service users To establish and maintain good working relationships with other community teams/agencies within the statutory voluntary and private sector.
