Responsibilities
Manage and oversee a portfolio of specialist cases, ensuring timely and appropriate interventions.
Collaborate with healthcare providers and multidisciplinary teams to develop and implement individualized care plans.
Monitor case progress and outcomes, adjusting strategies as necessary to optimize results.
Conduct comprehensive assessments to identify client needs and determine suitable support services.
Maintain accurate and up-to-date documentation of all case activities, following compliance standards.
Provide training and support to junior staff members regarding best practices in case management.
Analyze trends in specialist cases to identify areas for improvement and implement strategic initiatives.
Requirements
Bachelor's degree in social work, nursing, or a related field; Master's degree preferred.
Minimum of 5 years experience in case management or a similar role, with a focus on specialist cases.
Strong understanding of healthcare regulations and compliance standards.
Excellent communication and interpersonal skills, with the ability to build rapport with clients and professionals alike.
Proven problem-solving skills and the capacity to think critically under pressure.
Ability to manage multiple cases efficiently while prioritizing tasks effectively.
Proficiency in case management software and Microsoft Office Suite.
Hours Per Week: 35.00
Start Time: 09:00

Pay Per Hour: £32.54
