This post offers a great opportunity for a RMN/RGN or dual qualified nurse to utilise their specialist skills, working in a dedicated, friendly and supportive team. Weare looking for a compassionate, motivated and caring individual whom has experience in working with people with functional and organic mental illness with or without frailty.

The successful candidate will work as a member of the South East Essex Older Adult’s Community Mental Health Teamsas a physical health lead, combining pre-existing mental and physical health skills, in order to promote the health and wellbeing of our patients. This post offers the opportunity to help develop the existing initiatives in physical health monitoring and intervention, whilst being supported to be innovative in the way we offer holistic care. This role would allow the successful candidate to maintain the requirements for revalidation in both areas of practice

The post holder will be working collaboratively with other health care professionals, clinicians and other practitioners and close working with the community teams in the local neighbourhood model.

The post holder carries responsibility for the assessment of care needs and the development, implementation and evaluation of programmes of care for patients with Serious Mental Illness ( SMI )The delivery of expert skilled nursing assessments to patients whose needs are best met in the community including the patient’s own home or other care setting.

To work in partnership with health care professionals and other agencies to reduce the impact of poor physical health on the client group, improving outcomes.

To support the Community Mental Health Nurses in the physical health care of individuals with Serious Mental Illness (SMI).

To provide physical health assessments as the service require

To support other professionals in the service in undertaking physical health assessments.

To work collaboratively with primary health, develop and maintain links with other professionals.

To have accountability and responsibility for implementation of physical health pathways, to support the SMI physical health agenda.

To work across the service offering physical health support and advice to team members as required.

To support team members in providing care to complex cases where physical health needs are identified.

Working to develop an integrated approach to physical health assessment and intervention for patients with SMI and with Frailty. This role will include supporting wider Community Mental Health Nurses in providing care to complex cases where physical health needs are identified.

The post holder will develop working relationships with a range of professionals, services and organisations within South East Essex area. This includes primary care, Community Dementia Services , Frailty team, Community Nursing, Community Mental Health Teams, Social Care as well as Community based non-statutory services
