Crynodeb
Communication and Support: * Act as the primary point of contact for patient inquiries and concerns regarding the platform. * Develop user-friendly communication materials and resources to educate patients on utilising the platform effectively. * Provide ongoing support and training to users to enhance their confidence and comfort with technology. Collaboration with Stakeholders: * Work closely with local healthcare providers, primary care networks, and community organisations to integrate services and facilitate smooth collaboration. * Participate in regular meetings with stakeholders to update them on patient engagement activities and gather input for future initiatives. Reporting and Evaluation: * Collect and analyse data on patient engagement metrics, providing regular reports to the Project Director and other relevant stakeholders. * Identify trends and insights from patient feedback and engagement data to inform strategic direction and operational decisions. * Work closely with our evaluation partner Health Innovation Network. Qualifications * Proven experience in patient engagement, health promotion, or community services, preferably within a healthcare setting. * Strong understanding of chronic disease management, mental health, and community health initiatives. * Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations. * Experience with data analysis and reporting, with a focus on using patient feedback to drive improvement. * Familiarity with digital health technologies and a keen interest in user-centred design. * Ability to work independently and manage multiple tasks effectively in a fast-paced environment.
