We are looking for an enthusiastic and driven manager for Camden Memory Service (CMS). CMS is a speciality service that provides asessment of service users who are suspected of developing dementia.

In addition to providing a diagnostic service, CMS also provides follow-up to patients who have reveived a diagnosis of dementia. This follow-up includes medication, Cognitive Stimulation Therapy, psychological input and carer's support (START, Strategies for Relatives). All patients are reviewed regularly to check on the progress of their condition, and any concerns are escalated within the MDT.

CMS is a highly regarded memory service which is used as a best practice model for other memory services. Camden Services for Ageing and Mental Health are rated Outstanding by the CQC and there is a positive culture of quality improvement work and excellent integration with local physical health services including the frailty pathway in Camden.



The team enjoys good links with voluntary sector organisations and provides a single point of access for all people with dementia and their carers to contact.









The main duties of the post are:

· To have managerial accountability for the delivery of care to service users referred for assessment and treatment of cognitive disorders.

· Assume line management for memory service practitioners and administrative staff working within the Camden Memory Service.

· Be responsible for effective clinical and managerial communication systems to, from and within the Camden Memory Service including communication with the wider MDT and medical cover for the service.

· Provide expert clinical leadership, advice, support and liaison, education and training to other staff and agencies.

· Work in close collaboration with the Directorate, service users and community services to ensure best use of resources and management of the caseload.

· Work in collaboration with Barnet, Enfield, Islington and Haringey Memory Services

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.

The main responsibilities of the job are:

Provide professional, clinical, managerial and supervisory leadership to the Camden Memory Service.

Be responsible for screening referrals to the service for assessment and diagnosis and assume responsibility for the coordination of referrals to team members and their workload management.

Manage the diagnosis to end of life service and ensure that service users that have been diagnosed with dementia are reviewed on a regular basis and they are allocated appropriately for follow-up care.

Chair Camden Memory Service referral meetings, business meetings and service development forums

Ensure the Camden Memory Service is adequately and safely staffed within the agreed establishment.

Ensure a safe and therapeutic service environment, making efforts to minimize the number of Service User Safety Incidents

In the event of Service User Safety Incidents, ensure they are dealt with swiftly and in line with policy. Organise support to service users following incidents and debriefing of staff. Draw out learning points and areas for practice development.

Review incidents and investigate complaints, accidents and Service User Safety Incidents in accordance with agreed procedures and produce report and recommendations.

Ensure effective communication is established within the team and external departments, e.g., regular staff meetings, attendance at Head of Department meetings. OMG and appropriate Service Line meetings

Participate in the development of pathways within the Directorate to ensure joint working across services in the delivery of care to patients

Ensure effective implementation, monitoring and audit of all local and Trustwide policies e.g. complaints, disciplinary procedures, sickness/absence, Health & Safety and risk management.

Aim to establish local resolutions of all complaints, giving time to service users, relatives and others to express their concerns and working with the staff team to improve services to service users

Provide monthly reports to the Service Manager e.g. activity levels, case load, manpower and expenditure, staff issues and other information that is required.

Carry out any other duties as delegated by the Head of Service for the clinical area.
