The Paediatric Diabetes Team is a small MDT team who provide high standards of care for approximately 100 CYP with diabetes. The team are based within the Children’s Centre of Dorset County Hospital, the team, have close links with the inpatient ward, Paediatric Assessment Unit and medical team, providing experience and leadership in all aspects of paediatric diabetes nursing care across the paediatric workforce.
The post holder will provide specialist diabetes nursing care, working across inpatient, outpatient and the community setting throughout the West Dorset locality.
An autonomous practitioner, alongside the Paediatric Diabetes team, including clinical lead and medical colleagues, responsible for the assessment, primary and/or secondary diagnosis, management and evaluation of treatment and care for a caseload of children & young people aged 0-18 years with diabetes in both the hospital and community.
· The post holder will provide nursing care for a defined caseload, educating and supporting the child/young person and their family/carers in diabetes self-management skills from diagnosis to transition.
· Co-ordinate and implement an overall strategy of evidence-based care for the whole caseload of patients
· Co-ordinate the service alongside the Clinical Lead and lead practitioners for medicine, nursing, dietetics and psychology.
· Be a highly visible and accessible resource for specialist advice to professionals and carers providing care to children and young people with diabetes.

· Through effective leadership, ensure the clinical multidisciplinary team deliver a safe, effective, responsive, caring and well-led service where compassion is embedded, and a positive patient experience is achieved.
· Provide strong professional clinical and managerial leadership and act as a professional role model.
· Works across organisational and professional boundaries, including working collaboratively with health care professionals in the community and external agencies, for example social care, education and the voluntary sector.
· Act as an expert resource in Paediatric Diabetes with extensive specialist knowledge and clinical expertise to be an effective educator for health care professionals, CYP, their families and carers (including the education setting).
· Operate as Nursing Lead for Education overseen by the Nursing Leads. This incorporates the education requirements for ward staff, schools and patient education programmes.
· Work collaboratively with the Clinical Lead and service leads (e.g. medicine, dietetics and psychology to deliver the service to national standards such as the RCPCH National Diabetes Quality programme – currently PEER Review and BPT
· As a nurse prescriber (willing to undertake) demonstrate advanced knowledge of and experience in the use of Oral Hypoglycaemic Agents (OHA’s), insulin and other treatment regimens relating to diabetes.
· As a nurse prescriber advice on the stabilisation of patients on all oral agents, insulin, and other injected therapies according to national, regional, and locally formulated guidelines and protocols.
· As a nurse prescriber demonstrate advanced knowledge and experience in prescribing treatment within your scope of practice alongside local policy.
· You should possess the ability to perform assessments of patient needs and liaise with other members of the multi-disciplinary team where necessary.
