JOB DESCRIPTION 1. Job Details Job Title: Community Staff Nurse Location: Community Nursing Band: 5 Hours: Full Time, Part Time or Bank Hours Report to: District Nurse. Accountable to: Community Nursing Lead 2. Job Purpose Under the direction of the District Nurse or Senior Community Staff Nurse to provide high quality skilled nursing care, contributing to the ongoing assessment, planning, implementation and evaluation of the holistic care needs of the patient, family and/or carer. 3. Key Result Areas Provide and maintain a high standard of skilled nursing care for patients in their own homes, health centres or practice surgeries, in compliance with protocols and guidelines. Assess and re-assess patients with non-complex and complex conditions, reporting back to the District Nurse on any interventions undertaken. Respond effectively to the local practice population health needs through working within the primary health care and social care team. Promote health in all age groups by advising and supporting clients and families. Communicates observations of physical, mental and social conditions to the Integrated Team. Participates in health promotion activities in line with Yourhealthcare and local priorities. Participates in Clinical Governance activities such as clinical audit and research. Complies with current Evidence Based Guidelines, Yourhealthcare and relevant procedures. Demonstrates evidence based clinical interventions to pre-registration students while on placement with the nursing team. Reports to the District Nurse or Senior Community Staff Nurse any accidents, incidents or near misses to own person, the patient, patients property and equipment, in line with current policies. Professional Duties & Responsibilities To practice the principles of Community Nursing, maintaining a high standard of professional practice and competence in keeping with YH Standards, Policies and Guidelines. The NMC Code of professional Conduct, Scope of Professional Practice, Standards of Record Keeping. To maintain a professional portfolio of continuing professional development as required by the NMC and revalidate every 3 years. To participate in Case conferences and, meetings and ensure that all relevant information is exchanged To assist the District Nurse Team Leader in participating in the delivery of the NHS Plan, and public health agendas To participate in audit and data entry contributing to the Trusts targets To work within the clinical effectiveness framework, Essence of Care Education & Research To access annual individual performance review, identify training needs and develop a continuing professional development plan To assist with the induction, mentorship and teaching of newly qualified staff and colleagues. Student nurses To be a Practice Assessor on the local register To participate in working parties task forces and clinical governance and in service training as appropriate Keep up to date with current trends in community nursing, nursing research, clinical guidelines and continuing education programmes Communication To maintain high standards of professional practice and competence in keeping with that of the NMC and Trust standards for record keeping To communicate clearly and effectively with team members, multidisciplinary team and other agencies providing a seamless service To communicate with clients at a time and level appropriate to their needs in a manner that promotes positive health To act as an integral member of the primary health care team To maintain confidentiality with sensitive information when supporting clients in line with information governance policies. To be able to share information when appropriate To be able to communicate in a variety of ways which include, verbal, written, IT and report writing To be able to identify personal levels of stress and access service provided by the YH e.g. counselling services 4.
