Chelsea and Westminster Hospital NHS Foundation Trust is one of the leading providers of services for the LGBTQI+ communities including oursurgical service, Chelsea Centre for Gender Surgery (CCGS), thegender dysphoria clinic ‘TransPlus’, trans sexual health service ‘56T’. Our trans services are recognised for their inclusivity & diversity and our ambition is to be the leading healthcare provider for trans and non-binary service users in the UK, building on the specialist expertise and knowledge of our clinical teams.

Chelsea and Westminster Hospital has been commissioned by NHS England to provide a comprehensive gender surgery centre including masculinising lower surgery, masculinising chest and feminizing lower surgery. Our vision is for an innovative and patient-centred service, based on the principle that patients must have equal access to surgical services whatever their gender identity with reasonable adjustments to the delivery of care to match the individual’s needs and circumstances.

The post holder will have the following duties:

· Deliver outpatient clinics, diagnostic procedures and theatre lists to support the service

· Attendance at MDT meetings

· Participation in audit and governance activities of the department

· Ensure compliance with national commissioning standards and waiting time targets

· Support subspecialisation in the department by appropriate cross referral to colleagues

· Provide consultant care at an early stage to patients presenting to secondary care with presumed gynaecological problems

· Contribute to the care of GAS inpatients, rotating this responsibility with colleagues.

· To promote early and safe discharge of patients

· Strong commitment to the organisation and delivery of training.

We have nearly 7,000 members of staff that care for nearly one million people locally, regionally, nationally and internationally. Both hospitals provide full clinical services, including full maternity, emergency and children’s, in addition to a range of community-based services across London, such as sexual health and HIV clinics.
Our facilities are some of the best in the country. We have been investing around £10 million a year in our estate. We spent £25 million on state of the art adult and neonatal critical care facilities at Chelsea and Westminster in partnership with our charity, CW+ and generous donors. Last year alone we spent £7m on improved facilities at West Middlesex. Both hospitals have major A&E departments, treating over 300,000 patients each year. The Trust is the second largest maternity service in England, delivering over 11,000 babies every year. Our specialist care includes the world-renowned burns service, which is the leading centre in London and the South East; we run Chelsea Children’s Hospital with paediatric inpatient and outpatient services; and our specialist HIV and award-winning sexual health care services.

Roles and responsibilities

Research,Teaching andTraining

· To provide high quality teaching and mentorship in gender surgery to consultant surgeons, trainees and clinical fellows.

· Contribute to high quality teaching to medical undergraduates and other health care professionals.

· To collaborate with academic and clinical colleagues to enhance the Trust’s translational research portfolio, at all times meeting the full requirements of research governance.

· To provide high quality teaching to medical undergraduates and members of other health care professions as required by clinical director.

· To act as educational supervisor and appraiser as delegated by the divisional director/service director.

· Participate fully in continuing professional development to ensure that skills and knowledge are updated in line with relevant medical developments. In addition, consultants will be expected to undertake some non-clinical training and development at the discretion of the trust management, for example, Leadership Training.

· Excellent research opportunities are available throughout the Trust and with Imperial College. A number of funding opportunities are available from the Trust Research Committee and the Trust has experience in obtaining funds from among others the Department of Health, Medical Research Council, BBSRC, and the Welcome Trust.

· The Trust supports and has facilitated a large number of Honorary Academic appointments with Imperial College Faculty of Medicine. Opportunities should be discussed with the Divisional Medical Director/Service Lead/ Divisional Director of Operations.

PerformanceManagement

Thepost-holder isrequiredtoworkwithmedical,nursing,otherclinicalandmanagerialcolleaguesto ensurehighperformanceinthefollowingareas(where applicable):

· Clinical efficiency e.g. relating to surgical productivity and case mix of patients seen in clinic.

· Quality of outcomes e.g. number of patients readmitted due to complications within 3 months of discharge, number of revisions undertaken within 12 months of the primary procedure.

· Financial management e.g. identification, implementation and achievement of cost improvement programmes and participating in efforts to ensure services are provided cost effectively e.g. managing locum agency spend, monitoring and managing the drug budget to target, ensuring accuracy of clinical data for the team

· Operational efficiency e.g. day-case rates, waiting list activity and demand management.

MedicalStaffManagement andAccountability

· Work with colleagues to ensure junior doctors’ hours are compliant.

· Ensure that Trust systems and procedures are in place and followed to control and monitor leave for junior medical staff and to ensure that there is appropriate cover within the clinical areas, including on-call commitments.

· Adhere to Trust/department guidelines on leave, including requesting and reporting absence, and professional and study leave, and to ensure that members of staff for whom you are responsible do the same.

· Participate in the recruitment and induction of junior medical staff as delegated by the Clinical Director/Service Lead.

· Participate in team objective setting as part of the annual job planning cycle.

· Be responsible for ongoing assessment and regular appraisal of all doctors in training, Trust doctors and non-consultant grades as delegated by the Clinical Director/Service Lead /General Manager.

· Participate fully in regular departmental activities including department meetings and team building activities.

Governance

· Review clinical outcomes in designated area using external benchmarking data where appropriate, to identify and advise variances to the Clinical Director/Service Lead and participate in clinical audit, incident reporting and analysis and to ensure resulting actions are implemented.

· Work closely with the Directorate in relation to clinical and services developments as delegated by the Clinical Director/Service Lead.

· Participate in ensuring BAUS, NICE, NSF and other national guidance requirements are reviewed and implemented and monitored in the speciality areas.

· Ensure clinical guidelines and protocols are adhered to by junior medical staff and any other staff for whom the post has responsibility and that these are updated on a regular basis.

· Keep fully informed about best practice in speciality areas and ensure implications for practice changes are discussed with the Clinical Director/ Service Lead.

· Role model good practice for infection control to all members of the multidisciplinary team and adhere to the Trust Infection Control measures (available on the Trust Intranet).

· Report all clinical and non-clinical accidents or incidents promptly, and to co-operate accordingly with any investigations undertaken.

· Comply with the consent process outlined in the NHS England service specification for Gender Identity Services: care must be personalised and based on shared decision making with patients offered a choice of techniques and procedures which meet the eligibility criteria.

Strategy andBusiness Planning

· Work with clinical and managerial colleagues to support development of the gender dysphoria surgery service including the business planning and objective setting process for the directorate and Trust where appropriate.

· Represent the Trust at appropriate clinical networks/other external clinical meetings, as delegated by the Clinical Director/Service Lead.

· Attend and contribute at regular Departmental meetings as identified by the Clinical Lead.

LeadershipandTeam Working

· Demonstrate appropriate leadership skills with regard to individual performance, clinical teams, the Trust and when participating in national initiatives.

· Participate in any necessary management and leadership development courses as required by the trust.

· Work collaboratively with all members of the multi-disciplinary team, Imperial College London and other stakeholders as required.

· Set up and chair meetings relevant for clinical domain.

· Promote local resolution of any conflict or differences of opinion and involve appropriate parties for mediation, negotiation or discussion as appropriate.

· Provide commitment and leadership in relation to equality and appropriate senior level management of diversity issues, particularly as they apply to training, people management and service delivery.

· To adhere to Trust/departmental guidelines on leave including reporting absence.
