Falkirk Health and Social Care Partnership are looking to appoint a dynamic and enthusiastic Senior Home First Practitioner who will enjoy working in a busy environment within the Home First Team. This is a new role within the Home First team and an exciting time of team development and sustainable change within the service.

You will be educated to SVQ Level 3 in social care, CSS, Diploma in Social Work or equivalent. You will have excellent numeracy and literacy skills. Ideally you will also have knowledge and experience of local authority services, data protection legislative requirements as well as knowledge and understanding of the Home First ethos.

You should have experience within a health and/or social care setting and be able to demonstrate excellent leadership and management skills.

You will have a positive ‘can do’ attitude and the drive to ‘make it happen’ by being willing and able to contribute to a culture of ongoing improvement and a busy service. You must have excellent communication, supervision and IT skills, as well as being able to work to deadlines within a busy team.

The post is 37 hours and is based between Falkirk Community Hospital and Forth Valley Royal Hospital depending on demand.

This post is temporary until August 2025 due to service requirement.

If you are the successful candidate, you will be required to gain/maintain PVG scheme membership.
