Summary

To operate the Proton Therapy national referral portal to retrieve referrals, check with clinical site leads and list patients for MDT. Prepare and circulate lists of patients for discussion at each multi-disciplinary team meetings (MDT) using information provided by secretaries, lead clinicians and nurse specialists. To prepare and collate all the documentation, x-rays, pathology results/slides required to support each meeting, from referringTrusts across the country, thus enabling the smooth running of the MDT meeting. Using IEP and Sectra To attend the multi-disciplinary team meetings, keeping comprehensive records of attendance and outcomes of the meeting, and distributing to relevant team members. To ensure timely and accurate input of the outcomes of the MDT meetings onto local patient administration system, CWP To ensure that the necessary proformas are completed and entered into the relevant electronic systems. To ensure that all data entry of outcomes is recorded live within the MDT, or immediately after the MDT meeting. To check with the chairperson that all the information is correct and complete. To ensure that following the meeting all actions recommended by the team are followed up and that a record of the actions proposed has been filed in patient records. To ensure all MDTs are covered if there is any sickness, and to ensure that annual leave is covered by oneself or other members of the team. Work with ICT and AV suppliers for the maintenance of AV equipment at the Proton Centre and in the local network. To help facilitate the BT conferencing bookings, to amend test and liaise with external trusts to ensure the smooth running of the National Proton Therapy MDTs Collate relevant documentation, scans and approvals required from NHS referring centres to support each patient referred to the national Proton panel. Liaise with MDT/scheduling teams to ensure NHS patients are allocated to commissioned treatment centres appropriately according to the published NHS Standard Operating Procedure and NHS England PBT Clinical Policies. Resolve and respond to issues, incidents and queries, as and when required, in a timely manner from referring trusts across the county. Work closely with the National Clinical Lead to ensure timeliness of clinical reference panel and commissioning approvals to meet programme performance targets, when aiding /or covering the referral co-ordinator The post-holder will be expected to be diplomatic, self-motivated and enthusiastic with a professional attitude Additional Requirements Follow risk management procedures at all times, including the identification, assessment and management of risk within the division. To maintain awareness of and abide by all Trust policies and procedures. To be responsible for own professional/personal development, including participating in a quarterly appraisal and personal development planning. To ensure all the principles and practices associated with Caldicott and Data Protection legislation are rigorously adhered to.
