Crynodeb
To provide specialist, face-to-face screening at the point of referral to all those who are eligible. Complete a brief risk and mental state examination from the completed screening, making autonomous decisions around discharge and / or onward referral and signposting to primary care / mental health services / third-sector organisations if appropriate. Providing brief psychoeducation and interventions where appropriate. Acting as a senior clinician within the renal/kidney MDT; providing advice, clinical support, clinical supervision and senior review where necessary. Promoting and developing risk management and admission plans for those who may be transferred to another service. Participate in teaching and training of staff, as appropriate. Consistently apply effective decision making and problem-solving skills, including but not limited to the ability to prioritise workload, assess and manage mental health crises, manage conflict, work in a high-pressure environment and communicate across a range of services and professionals especially those working in the acute Trust. Maintain confidentiality by ensuring that information is only shared with those who need to know. Demonstrate a high level of interpersonal skills, self-awareness, leadership skills and empathy in all communications.
