Crynodeb
To represent Sandwell MBC at the Integrated Social Care & Health Centre by providing support to individuals to maximise their independence so that they can reach their full potential and return to a community setting. As a Reablement Care Assistant you will work with the customer through their person-centred plan, as directed by the Therapy Team. The aim is to maximise the customers independent living skills.
