Job Title: Interim Team Leader – Care Plan Champion
Salary: £37,500 per annum
Hours: 40 hours per week - Supernumerary – fixed term temporary contract for 6 weeks
Location: Edwinstowe, NG21 9PH

A little about Nightingale:
Nightingale Nursing & Care Home is based in the centre of Edwinstowe, near Mansfield. The home, although purpose-built, has been made very homely, and has a selection of garden areas, in line with Jasmine’s values and ethos. The home provides personal, nursing and dementia care to 49 residents.
Competencies for Interim Team Leader – Care Plan Champion:
- Excellent care planning skills
- Ability to perform and complete Mental Capacity Assessments
- Ability to complete Risk Assessments and Assessments
- Ability to work under pressure
- Good time management
- Excellent interpersonal skills
- Flexibility with working hours
- Good written and oral communication skills
- Ability to hold others to account
Day to Day Responsibilities for Interim Team Leader – Care Plan Champion:
- Report to the Home Manager
- Be accountable for ensuring all residents care plans are reviewed and meet the individual needs of the residents
- Ensure any actions allocated to the Senior or Nurse team are completed to a high standard
- You will ensure the care plan audit is followed to assure the Home Manager that the care plans are completed holistically and in a person centred way
Essential Requirements for Interim Team Leader – Care Plan Champion:
- Relevant Professional qualification (e.g. Registered Nurse, NVQ minimum level 3, care plan training)
- Experience of writing person centred care plans
- Experience of undertaking and recording Mental Capacity Assessments
- Ability to ensure information is triangulated across each residents care plan
- Live within 60 minutes of the Home
- Able to use a computer to record information within care plans
- Available for work immediately
Preferred Requirements for Interim Team Leader – Care:
- Experience as a Senior in Health or Social Care or above
