The Supported Pathway Officer is a critical role within our organization, designed to facilitate and promote pathways for individuals seeking support in various aspects of their lives. This position involves working closely with clients to assess their needs, develop tailored support plans, and connect them with appropriate resources. As a Supported Pathway Officer, you will be integral in advocating for clients, ensuring they receive the necessary guidance and assistance to navigate their personal and professional challenges. Your expertise will be crucial in identifying barriers to success and implementing solutions that empower individuals to take charge of their lives. You will engage with a diverse range of stakeholders, including government agencies, nonprofit organizations, and community groups, to create a network of support for your clients. This role requires a compassionate and proactive individual who can manage multiple cases effectively while maintaining a client-centered approach. Responsibilities
Conduct client assessments to determine individual needs and support requirements.
Develop and implement personalized support plans in collaboration with clients.
Connect clients with relevant local resources and services to address their needs.
Monitor progress and outcomes of support plans, making adjustments as necessary.
Advocate for clients in various settings, including meetings with other professionals and service providers.
Facilitate workshops and support groups to promote skill development and community engagement.
Maintain accurate and confidential records of client interactions and case progress.
Requirements
Bachelor's degree in social work, psychology, or a related field.
Proven experience in case management or support services is highly desirable.
Strong interpersonal and communication skills to effectively engage with clients and stakeholders.
Ability to work independently and as part of a multidisciplinary team.
Excellent organizational skills and the ability to manage multiple cases simultaneously.
Knowledge of local community resources and services available to support clients.
A compassionate and empathetic attitude towards individuals seeking assistance.

Pay Per Hour: £21.31
