Description

Responsibilities
Conduct assessments of clients' needs and develop tailored reablement plans.
Support clients in their physical and emotional rehabilitation journey.
Encourage and motivate clients to engage in daily activities to improve their independence.
Monitor and evaluate the progress of clients against their reablement goals.
Collaborate with healthcare professionals, social services, and other relevant agencies for comprehensive care.
Provide training and guidance to clients and their families to facilitate a smooth transition to independence.
Maintain accurate and up-to-date documentation of client progress and services provided.
Requirements
Hold a relevant qualification in health, social care, or a related field.
Demonstrable experience working in a care or support role, particularly with elderly or disabled individuals.
Excellent communication skills to effectively interact with clients, families, and professionals.
Strong assessment and analytical skills to evaluate client needs and progress.
Ability to develop and implement effective care plans tailored to individual client needs.
Demonstrate a compassionate and patient-centered approach to care.
Strong organizational skills and the ability to manage a varied and demanding workload.
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £16.00
