Summary

1. Accountable for own actions in accordance with The Code of Professional Standards of Practice and Behaviour. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times. 2. Effectively communicate information regarding clinical decisions, policy and care pathways to patients/carers and the multi-professional team. 3. Effectively communicate sensitive information to individuals from all backgrounds who are experiencing health inequalities and may be considered as vulnerable. The post holder will be working with patients who are receiving treatment from the locally commissioned Substance Misuse Service and will need to communicate serious diagnoses which will require significant patient engagement and lifestyle change to improve their outcomes. 4. A skilled practitioner and clinical leader providing support to healthcare professionals and Service Leads to ensure service objectives are met, participating in COPD Service / Policy development and involving patients and public. 5. Responsible for the clinical assessment of patients with COPD, ensuring evidence-based care is planned, implemented, and evaluated / audited. 6. Demonstrates highly technical and / or practical skills to ensure optimum patient management including complex patient monitoring; equipment management and frequent administration of invasive tests/procedures; for example spirometry, venepuncture, inhaler technique and capillary blood gas sampling. 7. Develop and educate staff, participating in supervision, mentorship and appraisal of students and junior staff. 8. Utilise and ensure others utilise IT systems to secure accurate and timely patient, workforce and resource data available. 9. Manage expected and unexpected clinical events requiring varying levels of physical effort according to patient dependency/clinical need. 10. Support patients, carers and others during difficult situations arising in the clinical area, e.g. imparting bad news or following an unexpected event. 11. Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
